Final Report Information
All organizations receiving
grants as part of annual compliance are required to submit a Final
Report. The due date is stated on Schedule A of
the contract; however, organizations will be notified by
mail or email about one month prior to the Final Report due date. For your
convenience, you may download the Final Report from this page and
email it when it is completed. Please note that all
requested supplemental material must be sent as part of the Final
Report (i.e. copies of
programs, brochures, posters, flyers, newspaper articles, etc.) They
may be scanned and emailed or mailed in the traditional fashion.
The Final Report will be considered incomplete without the
supplemental material.
REMEMBER THAT ELIGIBILITY FOR FUTURE FUNDING DEPENDS ON SATISFACTORY COMPLIANCE
AND COMPLETION OF THIS YEAR'S AGREEMENT, INCLUDING THE FINAL REPORT.
Organizations whose Final Reports are not received within two months
of the due date are ineligible for funding the following year.
